外周血miR-27a、miR-146a-5p水平与妊娠期糖尿病患者妊娠结局的关系OACSTPCD

Relationship between peripheral blood miR-27a and miR-146a-5p levels and pregnancy outcome in patients with gestational diabetes mellitus

目的 探究外周血微小核糖核酸-27 a(miR-27a)、微小核糖核酸-146a-5p(miR-146a-5p)水平与妊娠期糖尿病(GDM)患者妊娠结局的相关性.方法 回顾性选取2020年1月至2022年12月郑州大学第二附属医院妇产科接收的60例GDM患者当作研究对象,依照妊娠结局划分成不良组(n=29)、良好组(n=31),对两组临床资料行单因素分析与logistics多因素回归分析,采用Spearman相关性分析法分析外周血miR-27a、miR-146a-5p水平与妊娠结局的相关性,绘制受试者工作特征曲线(ROC)分析miR-27a、miR-146a-5p对GDM患者妊娠结局的预测价值.结果 两组总胆固醇(TC)、三酰甘油(TG)、miR-27a、miR-146a-5p比较差异有统计学意义(P<0.05);logistics多因素回归分析结果显示,miR-27a、miR-146a-5p为GDM患者发生不良妊娠结局的危险因素(P<0.05);Spearman相关性分析结果显示,外周血miR-27a水平与不良妊娠结局发生呈正相关,外周血miR-146a-5p水平与不良妊娠结局发生呈负相关(P<0.05);ROC曲线显示,miR-27a、miR-146a-5p预测GDM患者妊娠结局的曲线下面积(AUC)为0.915、0.902,联合预测的AUC最大,为0.979.结论 外周血miR-27a、miR-146a-5p水平与不良妊娠结局发生有关,且联合检测对妊娠结局具有较高预测价值,有助于GDM患者妊娠结局的早期判断.

Objective To ivnestigate the correlations between peripheral blood micro ribonucleic acid-27a(miR-27a)and micro ribonucleic acid-146a-5p(miR-146a-5p)levels and pregnancy outcomes in patients with gestation-al diabetes mellitus(GDM).Methods Sixty GDM patients admitted to the Department of Obstetrics and Gynecology at the Second Affiliated Hospital of Zhengzhou University from January 2020 to December 2022 were retrospectively selected as the study subjects.Based on pregnancy outcomes,they were divided into adverse outcome group(n=29)and good outcome group(n=31).Single-factor analysis and logistic multivariate regression analysis were performed on the clini-cal data of the two groups.Spearman correlation analysis was used to analyze the correlation between peripheral blood miR-27a,miR-146a-5p levels,and pregnancy outcomes.Receiver Operating Characteristic(ROC)curve analysis was conducted to assess the predictive value of miR-27a and miR-146a-5p for pregnancy outcomes in GDM patients.Results There were significant differences in TC,TG,miR-27a,and miR-146a-5p between the two groups(P<0.05).Logistic multivariate regression analysis revealed that miR-27a and miR-146a-5p were risk factors for adverse pregnancy outcomes in GDM patients(P<0.05).Spearman correlation analysis showed a positive correlation between pe-ripheral blood miR-27a levels and adverse pregnancy outcomes,while peripheral blood miR-146a-5p levels were neg-atively correlated with adverse pregnancy outcomes(P<0.05).The ROC curve indicated that the AUC for predicting pregnancy outcomes in GDM patients was 0.915 for miR-27a,0.902 for miR-146a-5p,and the AUC was maximized when combined,reaching 0.979.Conclusion Peripheral blood levels of miR-27a and miR-146a-5p are associated with adverse pregnancy outcomes,and their combined detection has high predictive value for pregnancy outcomes in GDM patients,facilitating early assessment of pregnancy outcomes.
